How to Request a Certificate
We have partnered with EOI Direct to manage all certificate requests. Please follow the steps below to obtain either a Master or Custom Certificate:
- Visit www.EOIDirect.com
- Click Register if you are a new user, or Login if you already have an account
- Select Evidence of Insurance
- Search for your association by name
- Follow the prompts to complete your request and download or receive your certificate

For Mortgage Lender or Escrow Company
Please be prepared to provide the following information:
- Name of the HOA
- Property address
- Name of the unit owner or borrower
- Loan number
- Mortgagee clause
For Unit Owners
If you are a unit owner within one of our HOA’s and need an annual renewal certificate or proof of insurance for your mortgage company, please have a copy of the letter from your lender ready to upload.
Understanding Certificate Types
Master Certificate
A Master Certificate provides a copy of the association’s master insurance policy and is typically used for informational purposes or association records.
Custom Certificate
A Custom Certificate includes specific borrower or lender details (such as loan number or mortgagee clause) and is generally required for property sales, refinancing, or escrow transactions.
